Forms are available on the Self-Help Resource Center and Legal Practice Forms pages in Adobe Acrobat (PDF) format. IMPORTANT: PDF viewers or readers other than Adobe may not work properly with our fillable pdf files.

Using Court Forms on Mobile Devices

Whether you are in the Apple or Android phone environment, all mobile devices open our forms as print only. In order to fill out our forms electronically, you will need to download Adobe Acrobat Reader on to your mobile device.


PDF Doesn't Open

  • Browser Issues:
    Chrome, Firefox, Safari, Microsoft Edge, etc. are now coming with their own PDF viewers, using them as a default instead of launching PDF files in Adobe Acrobat Reader DC. To check the settings on your browser use the instructions found on Adobe's website.
  • Using A Mobile Device:
    If you are trying to open a PDF file on a mobile device and it will not open, it may be a dynamic PDF file. Dynamic PDF files can expand or contract depending on the amount of text entered, and the user may be able to add and/or delete rows, while a static PDF file cannot be manipulated.
    • Dynamic PDF files will not open on mobile devices. Some of our dynamic forms are:

      • Volunteer Application
      • Civil Guardianship Reporting Forms
      • eCourts Access Request Form
      • CIC Improvement Grant Proposal Form
  • Using Older Versions of Adobe Reader:
    Adobe no longer supports older versions of the Reader software. Users who do not have a recent version of Adobe Acrobat Reader DC may experience error messages when accessing the PDF files on our site. To upgrade to the latest version of Adobe Acrobat Reader DC go to If you still experience difficulties after upgrading your software, please refer to the Acrobat Reader DC Learn & Support page on Adobe's website.

Content Appears to be Blank

Opening a PDF attachment in an email on the iPad (or iPhone), will open in iBooks (the default PDF viewer for Apple). iBooks will display the PDF as a blank, print only form, as it does not show the form fields. To see the data in the form, you need to open it in Adobe Acrobat DC.

  • Press and hold the attachment icon in the email.
  • A screen will appear that will allow you to select the option "Open in Adobe Acrobat".
  • Once the form opens in Adobe Acrobat, the data on the form will appear.

Data Not Saving

We have recently learned that there may be issues with our forms when using google chrome, and your data may not be saved. Google Chrome comes with its own PDF viewer, and no longer defaults to Adobe, which may cause issues.

If you are having issues saving your data you can set Chrome to default to Adobe, by following these steps:

  • Open a blank window in Chrome
  • In the address bar, type: chrome://plugins
  • A screen will appear that will that will show you the plugins you have on your machine
  • Disable the plugin for “Chrome PDF Viewer”
  • This will make Adobe your default PDF viewer, and you should no longer have issues when saving your data.
